Q: 3
Failure is _________

Discussion
Option A lines up with the ISTQB definition since a failure is about seeing the wrong system behavior from a fault. The other choices focus on when or where bugs are found, not what a failure is. Pretty sure A is it, but correct me if I'm off.
Had something like this in a mock and it matched option A. Failure is about incorrect behavior in the program because of a fault, not just when or where the bug is found. Pretty sure that's straight from ISTQB definitions.
B
Probably A
A is what I've seen in both the official guide and practice tests. Failure means the app does something wrong because of a defect, not just when bugs are found. Pretty sure that's ISTQB's standard-correct me if I'm wrong.
A is wrong, B and C are more about discovery timing, not what a failure is. The definition for failure in ISTQB is seeing incorrect program behavior because of a fault, which matches A. Pretty sure that's how the syllabus puts it. If someone has a different take, let me know.
Hard to say, A here
Pretty sure it's A
