I don’t think it’s C. B, D and A are needed for report access, not delivery or scheduling.
A B, D imo. Folder access (B) is what actually lets the sales manager see the report and field-level security (D) controls visibility of specific columns in the report. Sometimes you need a custom report type (A) if the standard one doesn't include all Shoe fields you want in the report. Not 100% since occasionally the default types are enough, but these three cover most real-world setups.
I don’t think it’s just B and D. A is needed too, since sometimes you have to create a custom report type to include all the Shoe object fields for the manager, not just rely on standard ones. Pretty sure about A, B, and D here-correct me if I missed something.

Maybe B, C, D. I thought report subscription (C) would help the manager get the report, since access and delivery sometimes blur together. Not fully confident though-A might be more relevant if a custom type is really required.
C/D? Custom report type (A) lets you pull in fields that standard types might skip. Folder access (B) is for letting the manager see the report at all, and field-level security (D) controls which fields show up. I think A, B, D makes sense but open if anyone disagrees.
A B, D. Report subscription (C) doesn't actually control access, it's the trap here.
