D imo, tips from employees or citizens is by far the most frequent way fraud gets detected. ACFE reports back that every year. Not saying it's always the best method, but for "most common," pretty sure D is right.

I get the stats about tips, but I'd actually pick B for this. Detective controls are built into the workflows and catch a lot regularly, so seems like a common way fraud gets picked up. Maybe I'm missing something obvious though, but C looks like more of a trap choice here.
